Test Code Billings Clinic: 4032 T4 (Thyroxine), Total Only, Plasma or Serum
Performing Laboratory
Billings Clinic Laboratory: Chemistry
Useful For
- Monitoring treatment with synthetic hormones (synthetic triiodothyronine will cause a low total thyroxine [T4])
- Monitoring treatment of hyperthyroidism with thiouracil and other antithyroid drugs
-
Total T4 levels offer a good index of thyroid function when the thyroid-binding globulin is normal and nonthyroidal illness is not present
Methodology
Direct Chemiluminescence
Specimen Requirements
Submit only 1 of the following specimens:
Serum or plasma should be physically separated from cells as soon as possible with a maximum limit of two hours from the time of collection.
Plasma
- Draw blood in a green-top (lithium heparin) tube(s).
- Spin down and send 1.0 mL of heparinized plasma frozen.
Serum
- Draw blood in a plain, red-top tube(s) or a serum gel tube(s).
- Spin down and send 1.0 mL of serum frozen

Interferences
The anticonvulsant drug phenytoin may interfere with total and free T4 levels due to competition for TBG binding sites.
