Methodology

Includes routine Papanicolaou staining and microscopic cytologic examination.
This test is useful for the detection of cervical cancer, its precursors, and other abnormalities of the female reproduction tract.

Note:  For Medicare beneficiaries, the ordering physician/nonphysician provider (NPP) must indicate the appropriate Pap selection on the “Gynecologic Cytology Request Form” in “Request Forms” in “Special Instructions.” The ordering physician/NPP must choose either screening Pap, high risk screening Pap, or diagnostic Pap based on the patient’s medical history documented in the patient’s medical record.

Note:  Please complete a “Gynecologic Cytology Request Form” in “Request Forms” in “Special Instructions” which includes indication of specimen source and all pertinent clinical data (clinical symptoms, history of malignancy, chemotherapy, radiation therapy). Selections on the requisition include a non-Medicare patient section and a Medicare patient section. The Medicare section requires the ordering physician or provider to choose from 3 Pap choices:  screening Pap, high risk screening Pap, and diagnostic Pap as well as PAP test selection. For all patients, the ordering physician or provider shall indicate test selections and include the diagnosis code.

For Medicare patients, an Advanced Beneficiary Notice must be obtained when appropriate. See “Advanced Beneficiary Notice (ABN)—Form” in “Medical Necessity and Advanced Beneficiary Notice (ABN) Policy and Form” in “Special Instructions.”
